What's the JavaScript error that's given when viewing this page? The menu is not displaying which could be causing the issue.

Fixed the ugly layout bit by changing <div jwcid="@If" to <span jwcid="@If".

As far as the JavaScript error, I don't know what's causing this. It doesn't happen on signup.html and the pages' form are very similar. I viewed-source, saved them and diffed them but didn't see anything that could be causing this.
